Cocaine has been surpassed by heroin as the main threat posed to Massachusetts, after showing less of a threat throughout much of the 1990s. Heroin's increasing popularity is due in large part to a significant decline in price and a rise in popularity. Heroin appears to be a greater problem than cocaine in high density locations; law enforcement authorities in all counties with more than 500,000 population (Suffolk, Essex, Middlesex, Worcester, Norfolk, and Bristol) identify heroin as their greatest drug scourge. In 1000’s of statewide substance abuse help-line calls in which drugs were mentioned, heroin was specified in 23 percent of calls between May and September of 1999, cocaine in 17 percent. The percentage of heroin related calls was consistent with the previous 5 month period.

Overdoses due to Heroin have become a significant problem in the state of Massachusetts. The Boston Medical Center reports that heroin-related overdoses are on the and police data and news reports from other cities in Massachusetts indicate that overdoses have shown an increase across the state. The city of Lynn experienced notable increases in the amount of heroin deaths prior to 1999. Thirty fatalities and 206 non-fatalities were recorded in Lynn from 1996 to late 1999, and the rate of deaths from heroin was double the area’s rate of homicides in 1999.

There are tell-tale signs of heroin use. If the heroin addict is allowed to smoke tobacco cigarettes in the house, the contents of the ashtray will be very different from non- addict smokers. There will be loose and unburned tobacco. The reason is a little bit of tobacco is removed from the cigarette to create space for the brown sugar to be added. If the addict is chasing the substance, one can notice matchsticks much more in proportion to the cigarette butts. The match sticks will be burnt to the end. If the heroin is smoked in cigarettes, then one finds that the cigarette is smoked down to the filter. "We don't even waste a speck of smack", is the explanation.
